Emergency Contact Information/Physically Challenged Occupants

An Updated Population Census & Contacts form is available here which asks that you list the name of any physically challenged occupants of your suite along with the type of assistance required, in the event of an emergency.

Additionally, you are requested to indicate emergency contact names for after hours emergencies. It is very important that we have the names and telephone numbers of people to contact in the event of an after hours emergency in the building.
